The Preventive/Comprehensive Cardiology practice utilizes a unique team approach to the primary and secondary prevention of heart diseases. In addition, it is a rapidly expanding area dedicated to providing care to patients with a broad spectrum of cardiology needs. The Nurse Practitioner/Physician Assistant in this role will be integral to managing a diverse patient population, utilizing a team-based approach to deliver high-quality, evidence-based care. This position offers the opportunity to work collaboratively with MD support while also exercising professional autonomy. Key Responsibilities:
- Conduct thorough assessments of patients with cardiovascular conditions.
- Educating and counseling individuals and families on cardiovascular disease prevention and wellness.
- Develop and implement individualized treatment plans with focus on lipid management, blood pressure management and weight loss pharmacologic therapy.
- Provide ongoing care and follow-up for patients with chronic heart conditions with emphasis on coronary disease and management of risk factors.
- Collaborate with multidisciplinary teams to coordinate comprehensive care.
- Diagnose and manage acute and chronic cardiac conditions.
